What is ISGH Health Access Card Program?
ISGH Health Access Card Program is a community-driven program offered by the Islamic Society of Greater Houston (ISGH), one of the largest Islamic organizations in North America. We facilitate access for individuals and families with a particularly those in financial hardships network of physicians and clinics offering reduced-cost healthcare services across the Greater Houston area.
Who is eligible to apply for healthcare assistance?
Our program is open to all community members particularly those who demonstrate financial need. Eligibility is based on household income, family size, and residency in the Greater Houston area. You must be a member of ISGH to apply.
Applying for Care
How do I apply for healthcare access card?
Click the “Apply Now” button in the top navigation, fill out the multi-step application form with your personal, household, and basic medical information along with required documents then submit. Our team will review your application and contact you within 10-14 business days.
What documents will I need to apply?
You’ll typically need a valid photo ID, proof of ISGH membership, proof of household income (recent pay stubs, bank statements, tax return, or benefits letter), Don’t worry if you’re missing something — our coordinators will guide you.
How long does the application process take?
Most applications are reviewed within 10-14 business days. Once approved, you can begin scheduling appointments with providers in our network right away.
Is there a fee to apply?
No. Applying is completely free. There are no application fees, processing charges, or hidden costs at any stage of our program.
Finding & Visiting a Provider
How do I find a provider near me?
Use the “Find a Provider” page to search our directory by doctor name, specialty, clinic, or address. You can also filter by specialty (Family Medicine, Cardiology, Dental, Pediatrics, etc.) and click the phone or directions icon to contact or navigate to the clinic.
What types of specialties are available in the network?
Our network includes Primary Care, Internal Medicine, Family Medicine, Pediatrics, Cardiology, Dental Care, Nephrology, Psychiatry, Gastroenterology, General & Bariatric Surgery, Pulmonology, Podiatry, Vascular Surgery, and more — covering most routine and specialty care needs.
Do I need a referral to see a specialist?
For most specialties in our network, you can book directly. However, some specialists may request a referral or recent records from your primary care doctor. You can reach out to the card team in case of any issues booking an appointment
Privacy & Cost
Is my information kept confidential?
Yes. All information are handled with strict confidentiality. We work only with HIPAA-compliant providers and never share your personal data without your consent.
How much will my visits cost?
Most services are offered at a significantly reduced rate, depending on the provider and your eligibility tier. You’ll receive a clear cost breakdown before any appointment — there are no surprise bills.
Will it cover prescriptions or lab tests?
Coverage for prescriptions, lab work, and imaging varies by provider and case. Many of our volunteer clinics offer discounted lab and pharmacy services.

Can I get help filling out the application?
Absolutely. If you have trouble with the online form, visit our Contact Us page or email info@isghhealthcare.org to schedule assistance — either in person at an ISGH center, by phone, or via video call. We have multilingual coordinators available.
